We have made requesting an appointment (optional) a simple process via our Web site. We will contact you to verify appointment availability. If for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.

Insurance and Billing
We accept most insurance plans such as: Aetna, Auxient, BCBS, Cigna, Cypress, Delta Dental, Gaurdian, Health Benefit Fund, Humana, MetLife, Milwaukee Carpenters District, NTCA, Physicians Mutual, Principal Life Insurance Company, SunLife Financial, TeamCare, United Concordia, UnitedHealthcare, Wisconsin Electrical Employees and many more. Contact our office if your insurance is not listed to verify acceptance of your plan. We will be happy to file your insurance claims for you, as long as you are free to choose your own dentist. We are not a provider for Badgercare or Forward Health.

Payment Options
We accept checks, cash, credit/debit card and CareCredit. Payments are expected in full with in 60 days after insurance pays and with in 90 days for patients without insurance. After this time, if no payment is made, you will be informed of your past due account with a courtesy call. If our attempts to contact you are not succesfull, and or payment arrangements are not made, your account will unfortunately need to be turned over to collections. Fortunately, most of our patients see the value of our care, and this is rare.
